Network Health Plan is a regional health care organization that provides services to more than 75,000 members of over 1,400 employer groups throughout northeastern Wisconsin. It provides comprehensive health care coverage through partnerships with various medical facilities, physicians, pharmacies and other health care providers. It focuses on community-based care, including outpatient, inpatient, ancillary, long-term care and insurance services offered to the patients. Network Health Plan is approved by the National Committee for Quality Assurance. It offers various employer health plans, including health maintenance organization, point-of-service, preferred provider organization and consumer-driven health plans. It is a part of Affinity Health System, which is a Catholic mission-oriented regional health care network. Network Health Plan has its office in Menasha, Wis.
