Bayshore Health Care & Rehabilitation Center, based in Holmdel, N.J., is a Medicare- and Medicaid-approved skilled nursing facility with 190 beds, ventilator unit and a subacute care unit. A 50-bed personal care home is also present in the center s building. It houses a subacute care unit that is supported by its modern rehabilitation therapy department and staffed by a team consisting of physical and occupational therapists, speech and language pathologists and physiatrists. It offers semiprivate rooms with bathrooms and also offers private accommodations. Three meals are served daily in its dining rooms and in-room service is provided if needed. The center also has a team of registered and licensed private nurses and certified nursing assistants to monitor medications and develop health care plans to coordinate, care and assist with activities of daily living, such as bathing, dressing, walking and eating. The center also provides transportation services, off-site physician and diagnostic services.
