Susan Diaz, BSN, RN, MS, CNM received her bachelor of science degree in nursing and her master of science degree in nursing and midwifery education from the University of Illinois at Chicago. During her clinical rotations at Carle Hospital, Greenville Regional Hospital and the University of Illinois Medical Center, Susana provided womens health and obstetrical services and education for women of various ethnic and social groups. She is a member of the American College of Nurse-Midwives and the American Congress of Obstetricians and Gynecologists. Susana is a native Spanish speaker and is proud to serve the Latino community in Michiana. She is focused on promoting midwifery care and providing excellent obstetrical, gynecological and primary health care services for women across their lifespan.
M. Shakil Aslam, MD, MPH, FACC, FSCAI received his medical degree from King Edward Medical University in Lahore, Pakistan and Masters in Public Health / Health Administration from University of Oklahoma-health Sciences Center, Oklahoma City. He completed his internal medicine residency at Chicago Medical School, Rosalind Franklin University, cardiology fellowship at Advocate Lutheran General Hospital, Park Ridge, Illinois and Interventional cardiology fellowship at Aurora St. Lukes Hospital / University of Wisconsin Milwaukee Affiliated Hospitals in Milwaukee, Wisconsin. He is board-certified in internal medicine, cardiology, nuclear medicine, interventional cardiology and endovascular medicine. He is a fellow of the American College of Cardiology and a fellow of the society of cardiac angiography and intervention. He serves as faculty at University of Indiana School of Medicine as Voluntary assistant Professor. He is currently director of Cardiac Cath Lab at Memorial Hospital of South Bend. He serves as a member of the Governing Council of the Memorial Leighton Heart & Vascular Center and various quality and peer review committees. He formerly served as director of Cardiac Rehabilitation at Memorial Hospital of South Bend and director of ICU and Cardiopulmonary Services at Grandview Hospital in Michigan He has special interest in coronary, peripheral and endovascular interventions, transcatheter aortic valve replacement (TAVR), as well as structural heart diseases.
Nicholas H. Kerr, DO received his medical degree from Des Moines University College of Osteopathic Medicine in Des Moines, Iowa, and completed his family residency at Saint Joseph Health System in Mishawaka and his hospice and palliative medicine fellowship at the University of Louisville School of Medicine. He is certified by the American Board of Family Medicine. Dr. Kerr is a member of the American Academy of Family Physicians and the American Academy of Hospice and Palliative Medicine. Dr. Kerr chose to specialize in palliative medicine to fulfill his passion to provide comprehensive supportive care and assistance to patients and patients families facing both life altering and terminal illnesses. Dr. Kerr lives in South Bend with his partner Jesse and their two rat terriers, Betty and Camille. He enjoys spending time with family and friends, cheering on the Irish, trying new locally owned restaurants, and living life to the fullest.

From a young age, Dr. Hari Gourabathini was inspired by his father, who was also a physician. His father was honored and respected in the community for his work saving lives, and his young son decided he wanted to impact lives in the same way. In particular, he wanted to work with children in the Intensive Care Unit (ICU). “When you treat a child, you don’t just make them happy,” he said. “You make their whole family happy. ” Dr. Gourabathini strives to treat every child as if they were his own, and he wants patients and their families to leave knowing that he was caring, honest and trustworthy. “It’s especially gratifying when someone comes back to the ICU after you save them,” Dr. Gourabathini said. “It’s inspiring to save them, and see how your hard work has paid off. That makes a huge difference.” He’s especially thankful for the team he will be working with, who he’s said are skillful and capable people, the best colleagues he could ask for starting a new job. Outside of work, he enjoys playing tennis, swimming, and spending time with his own children.
Stephen M. Smith, MD received his medical degree from Indiana University School of Medicine and completed his neurosurgical residency at the University of Kansas in 2004. He completed a subspecialty fellowship training in spinal surgery at the University of Tennessee Health Sciences Department. Dr. Smith has had extensive training in minimally invasive techniques for discectomy, decompression and fusion. His background offers experience with motion-preserving technologies including artificial disc replacement, vertebroplasty/kyphoplasty and minimally invasive foraminotomy. He is also skilled in surgical procedures for spinal tumors, vascular malformations and traumatic spinal fracture. He is an active member of the North American Spine Society, the American Association of Neurological Surgeons and the Congress of Neurological Surgeons.
Walter S. Langheinrich, MD received his medical degree from Indiana University School of Medicine and completed his surgical internship and neurosurgical residency at the University of Pittsburgh Medical Center. He obtained his board certification from the American Board of Neurological Surgery in 2000. His expertise is well respected among colleagues, and he has published several articles in periodicals such as Internal Medicine and the Journal of Neurosurgery. His career also includes extensive research in the radiobiology of human meningioma. Dr. Langheinrich?s special interests are: microsurgical treatment for trigeminal neuralgia, vagal nerve stimulator treatment of epilepsy, baclofen and intrathecal pump therapy for spasticity, spine and tumor surgery.

The South Bend Clinic and SurgiCenter was founded in 1916 as a multi specialty physician partnership. Its founding was inspired by a visit to the Mayo Clinic by Dr. Walter Baker, a South Bend surgeon. Upon his return, he gathered six other specialists to form the clinic. Since its establishment, the clinic has provided both primary and specialized care. Samuel McGrath, MD, is board certified radiation oncologist who has been practicing at Memorial Hospital of South Bend since 2010. He completed his internship and residency at William Beaumont Hospital in Royal Oak, Michigan. While at William Beaumont, Dr. McGrath had the opportunity to train under several nationally renowned radiation oncologists, acquiring valuable experience with advanced treatment planning and delivery techniques, including radiosurgery, brachytherapy and IMRT/IGRT. He received his medical degree from Wayne State University School of Medicine in Detroit, Michigan, graduating with highest distinction as a member of the Alpha Omega Alpha national medical honor society. Prior to this, he completed his bachelors degree in chemistry at Indiana University, Bloomington, where he graduated summa cum laude and as a member of the Phi Beta Kappa honor society. Dr. McGrath has published on a variety of topics, including prostate, breast and lung cancer. He is an active member of the American Society of Therapeutic Radiology and Oncology.

The Womens Care Center is one of the largest pregnancy resource centers in the United States. It offers ultrasound, supportive counseling, 24-hour hot line, and pregnancy testing and verification services. The center maintains more than 15 locations in northern Indiana, southwestern Michigan and central Ohio. The Womens Care Center serves over 9,000 women each year. It provides parenting classes and referrals for medical and social service organizations. During your visit to the Anticoagulation Clinic, we check your blood to see if you have the right amount of medication in your body. All that is needed is a small amount of blood from a finger stick. There is no need to go to a separate lab facility and no need for huge needles. For your first appointment at the Memorial Hospital Anticoagulation Clinic, please park in the Bartlett Street Parking Garage or use our convenient valet parking service. Enter through the Main Entrance of the Hospital. You will need to check in at the Registration-Admitting Office, located just past the South Bend Chocolate Company, at least 15 minutes prior to your first appointment only.  Once you have checked in, please follow the signs to reach the Anticoagulation Clinic or ask to have a People-Mover drive you to the Anticoagulation Clinic location.
